This referral pathway has been specifically designed for the North London Foundation Trust (NLFT) and should only be used by NLFT staff.

Why do you need the hospital, ward or team details?
This allows us to understand where the referral has come from and how we might support the carer.
Why do you need the patients address?
We want to make sure that your information is sent to the right carer support organisation. There are five working across North Central London and we use the postcode to ensure the carers details arrive with the right one.
I do not know the discharge date, does it matter?
No. We appreciate that there is often not a fixed date. It just helps us to plan if you do know.
I don’t know what support I am going to need, does it matter?
Of course not. That is what we are here to help the carer with. Sometimes they would have been told about equipment and other things and it is useful for us to know these things in order to help the carer.
Why do you need to know the carers relationship to the patient?
They don’t have to tell us but this information can help us to help you when we are looking at the support you need with the discharge plan.
Why should you refer an unpaid carer for support? What support can unpaid carers get?
Through the North Central London Carers Support Project, you can now quickly and easily refer carers for support from their local carers organisation. Local carers organisations offer a variety of support to carers to support them in their caring roles.

What happens next?
The carer will immediately get a copy of the registration via the email address you have provided. The information will be sent securely to one of the five Carer Support Organisations that are working in North Central London. They will then contact the carer within 48 hours of the registration being received. The carer will be given details of the support that is available to them in their area and about the support they can expect around the hospital discharge process.
