This referral pathway has been specifically designed for the London Ambulance Service (LAS) and should only be used by LAS staff.

You can refer more than one carer if necessary.

What happens next?
The carer will immediately get a copy of the registration via the email address you have provided. The information will be sent securely to one of the five Carer Support Organisations that are working in North Central London. They will then contact the carer within 48 hours of the registration being received. The carer will be given details of the support that is available to them in their area and about the support they can expect around the hospital discharge process.
