Why do you need the hospital and ward details?
This allows us to understand what services might be available to you in the hospital and to help support you with the discharge process.
Why do you need the patient’s address?
We want to make sure that your information is sent to the right carer support organisation. There are five working across North Central London and we use the patient address as well as your own to ensure your details arrive with the right one.
I do not know the discharge date, does it matter?
No. We appreciate that there is often not a fixed date. If you do know, it helps us to plan for the support that you may need.
I don’t know what support I am going to need, does it matter?
Of course not. That is what we are here to help you with. When your local carers’ organisation receive your referral, they will be able to offer you support dependant on your situation.
Why do you need to know my relationship to the patient?
You don’t have to tell us but this information can help us to help you when we are looking at the support you need with the discharge plan.
