If the person being cared for is due to be discharged soon, please use this rapid registration form and we will contact you as soon as possible. For this rapid registration, we only require your name, postcode, the postcode of the person you care for and your contact email and phone number.
Someone from the appropriate carers support organisation will be in contact with you within 48 hours or sooner.

What happens next?
The carer will immediately get a copy of the registration via the email address you have provided. The information will be sent securely to one of the five Carer Support Organisations that are working in North Central London. They will then contact the carer within 48 hours of the registration being received. The carer will be given details of the support that is available to them in their area and about the support they can expect around the hospital discharge process.
