We know that having someone discharged from hospital into your care can be daunting. But you are not on your own. The hospital staff will be able to help you and there are carer support organisations in Barnet, Camden, Enfield, Haringey and Islington who can give you the support , help and information that you might need.

Why do you need the hospital and ward details?
This allows us to understand what services might be available to you in the hospital and to help support you with the discharge process.
Why do you need the patients address?
We want to make sure that your information is sent to the right carer support organisation. There are five working across North Central London and we use the postcode to ensure your details arrive with the right one.
I do not know the discharge date, does it matter?
No. We appreciate that there is often not a fixed date. It just helps us to plan if you do know. We will be in contact with you soon.
I don’t know what support I am going to need, does it matter?
Of course not. That is what we are here to help you with. Sometimes you would have been told about equipment and other things and it is useful for us to know these things in order to help you.
Why do you need to know my relationship to the patient?
You don’t have to tell us but this information can help us to help you when we are looking at the support you need with the discharge plan.
What happens next?
You will immediately get a copy of the details you have filled in on this form via the email address you have provided. Your information will be sent securely to one of the five Carer Support Organisations that are working in North Central London. They will then contact you in the next couple of days to give you details of the support that is available to you as a carer in your area and about the support you can expect around the hospital discharge process.
