We know that the Allied Health Professionals in hospitals are often very involved with a patient discharge and can be key to identifying carers involved in the support of the patient.
Use the simple form below to give us a little bit more information and the carer will be contacted by the local support organisation based on their postcode. You can refer more than one carer if necessary.

Why do you need the hospital, ward or team details?
This allows us to understand where the referral has come from and how we might support the carer.
Why do you need the patients address?
We want to make sure that your information is sent to the right carer support organisation. There are five working across North Central London and we use the postcode to ensure the carers details arrive with the right one. For those carers who live outside of the 5 London Boroughs, we will signpost them to their local carers support organisation.
I do not know the discharge date, does it matter?
No. We appreciate that there is often not a fixed date. It just helps us to plan if you do know. It may also be that the patient has already been discharged from hospital.
Why do you need to know the carers relationship to the patient?
They don’t have to tell us but this information can help us when we are looking at the support that is needed.
What happens next?

The carer will immediately get a copy of the registration via the email address you have provided. The information will be sent securely to one of the five Carer Support Organisations that are working in North Central London. They will then contact the carer within 48 hours of the registration being received. The carer will be given details of the support that is available to them in their area and about the support they can expect around the hospital discharge process.
